This complaint form is to be used for registering a complaint regarding a food establishment, tattoo facility, hotel/motel, public health or smoking nuisance with Allen County. The Department will respond to complaints as soon as possible. When submitting a complaint, please include specific information as to the nature and location of the complaint and any issues observed. If specific information is not provided, an inspection many not occur. IF REPETITIVE UNSUBTANTIATED COMPLAINTS CONTAINING INVALID INFORMATION ARE RECEIVED, AN INSPECTION WILL LIKELY NOT TAKE PLACE as the Department will use discretion on what requires attention.
